I visited his page.... damn ! The man does such expensive and challenging work, has a reputation as a good craftsman and yet has allowed such a ridiculous situation with feedback and debts to clients.
If your business is expanding and this is your goal, you need to have workers at your disposal, or at least a person on the phone/email...or a bot like on some sites.
Or, don't take as many prepaid orders. It's such a risk !

Thank you, will do that. The other option is to escalate a dispute via PayPal and highlight as a scam. But, if this is genuinely a health issue, I don't want to cause further problems. I also do just want to get my watch back, which I think he has done an amazing job with.
