I wonder if you are referring to the Castle Bravo event ? The US gov miscalculated the fusion potential of the lithium isotope mix, resulting a blast several times more powerful than the theoretical maximum predicted yield. It resulted in a massive radiologic disaster too.
The Russian 'Tsar Bomba' was the largest yield weapon ever detonated, and also the cleanest based on fallout/yield... had they used an fissionable tamper it would have been about double the yield with orders of magnitude more fallout. Luckily most fission fallout decays fairly rapidly; I-131 is a major product which I use almost daily at work - it is the 'magic bullet' that achieves a 99% cure rate for common forms of thyroid cancer. Also it's now being tagged to various monoclonal antibodies to fight other cancers.
Anyone interested in this stuff should watch the old documentary 'Trinity and Beyond'... it's fascinating and worthwhile history lesson.
