Well they dont need to be on 12 o'clock. But you definitely need to make sure that they are lining up with the hour markers etc. As long as when you set them the hour hand is on an hour marker and the minute hand is on the 12 marker then things will continue to be aligned. The seconds hand you can of course replace anywhere you want.

Just my 2¢, I agree that the better tool for hand removal would be the lever style. I've tried the other and just couldn't get used to them. Of course I should probably mention I'm missing two main fingertips on my right hand...

As far as the all at 12 o'clock hand setting, nothing better than starting a good habit. In this video, they show how the watchmaker installs the hands at 12- but FAIL! This $100,000 watch still doesn't change the date at midnight! Start at 5:12....
