It is an SKX031 sometimes called the Seiko 'Submariner'

Sadly it is no longer in production, but they come up on eBay quite often. There is also a Pepsi version.

It is not a ceramic insert, but they are available, as are other mods like domed crystal...you name it, people go nuts for modding Seikos.
